Component Task: Home Visit to a Pregnant Woman

This is a visit made by the Public/Community health nurse to a pregnant woman at home.

Aims

·         Monitor and supervise foetal growth and maternal wellbeing

·         Detect pregnancy related conditions for immediate intervention

·         Counsel and educate the mother on birth preparedness plan

·         Ensure compliance to antenatal visits

·         Educate client on stress management

·         Counsel and educate client and husband on sexual activities

·         Encourage partner and family support

·         Encourage client to take her routine medications

·         Encourage client to report any abnormality for prompt intervention

Requirements

·         Refer to items in the well-equipped home visiting bag

·         Map of the area/community

·         Records of the client manually/electronically

·         Register of the area/community

·         Arrange for transportation

·         Vaccines in a vaccine carrier

·         Safety box for used sharps

Steps

1.        Read map of the community

2.      Knock, enter, greet and introduce self

3.      Ask about general health

4.      Observe environment for cleanliness, safety and comment

5.      Request for the maternal and child health record booklet and find out about clinic attendance

6.      Assist client to identify health needs

7.       Check and use items in home visit bag

8.      Wash hands/alcohol hand rub with appropriate technique

9.      Explain procedure and checks vital signs and weight

10.    Perform general physical examination (Head to toe)

11.      Inspect and palpate abdomen (if possible)

12.    Wash hands/alcohol hand rub with appropriate technique and counsel client according to findings

13.    Interview and counsel on:

a.      Diet

b.      Bowel movement

c.      Rest and sleep pattern

d.      Exercises

e.      Birth preparedness plan

f.        Healthy timing and spacing of pregnancy

g.      Danger signs of pregnancy per maternal and child health record book

h.      Prevention of malaria with Sulfadoxine Pyrimethamine (SP) and the use of Insecticide Treated Bed net (ITN)

i.         Early reporting when labour starts

j.         Male involvement

k.       Regular attendance to antenatal clinic

14.    Record and report findings manually/electronically
