Component Task: Home Visit to a Motherless Baby

This is a visit made to a motherless baby and the caregiver by the Public/Community health nurse at home.

Aims

·         Counsel caregiver on hygienic care of the baby to prevent infections

·         Teach caregiver on proper feeding to increase warmth, promote growth and development

·         Detect any deviation from normal for prompt attention

·         Encourage caregiver to attend CWC regularly

Steps

1.        Read map of the community

2.      Knock, enter, greet and introduce self

3.      Establish rapport and explain the purpose of the visit to care giver

4.      Observe environment for cleanliness, safety and comment

5.      Assist client to identify health needs

6.      Check and use items in home visit bag

7.       Wash hands/alcohol hand rub with appropriate technique

8.      Explain the procedure and examine the child from head to toe

9.      Wash hands/alcohol hand rub with appropriate technique

10.    Interview and counsel on:

·         The child’s general health

·         Replacement/complementary feeding and care of the feeding utensils

·         Provision of warmth/clothing

·         Prevention of infection/personal cleanliness

·         Sleeping pattern of the child

·         Immunization status, CWC attendance, etc. as per child health record book

11.      Explain findings to the care giver

12.    Encourage attendance at child welfare clinic, congratulates and schedule for next visit

13.    Record and report findings manually/electronically
