This is a visit to the aged (above 65 years) in his/her home environment to render service based on their health needs. E.g., physical, psychological, social and emotional needs.

Steps
1. Read map of the community to identify the home of the client and family
2. Enter home and greets according to custom
3. Introduce self to client and family
4. Ensure privacy and confidentiality
5. Observe environment for cleanliness, safety and comment
6. Check client’s previous records
7. Assist client to identify needs/special problems
8. Ask questions
9. Check and use items in home visit bag
10. Check vitals of clients e.g., temperature, pulse, respiration, blood pressure
11. Perform physical examination
12. Interview and counsel client on:
· Diet
· Bowel movement and personal cleanliness
· Mobility/exercise and sleep pattern
· Recreational activities
· Home help (caretaker)
· Regular medical check/adherence to medication
· Prevention of home accidents
· Good interpersonal relationship with family members/socialization
· Initiation/continuation of any special activity
13. Assist in providing any care required
14. Congratulate client and caretaker and schedule next visit with them
15. Record/report findings to client and caretaker if any
