This is a therapy in mental health care where the RCPN recommends the referral and or facilitate the admission of a client in the community who has recovered from mental illness including alcohol and or substance use to a rehabilitation centre. This therapy may also be applicable to clients who have suffered from sexual disorders for them to benefit from proper human sexual rights and family values

Steps
1. Introduce self and establish rapport (refer to procedure)
2. Read map and or GPS of the rehab centre
3. Takes demographic data
4. Establish clients diagnosis
5. Identify client strength and challenges
6. Assess clients support system
7. Plans with the family on the rehabilitation processes
8. Assess previous occupational history
9. Identifies client’s current occupational potentials and interests
10. Determine if client can continue with the previous occupation or needs re-releasing or designation of occupation
11. Liaise with the occupational therapist and or the psychiatric social worker to identify rehabilitation centers within clients community or any other job opportunities
12. Link client to an identified rehabilitation center
13. Identifies and links client to social support system such as NGO’s or individuals who can be of help
14. Encourage client to join and participate in social activities such as religious groupings or social clubs
15. Ensure continuity of care and medication compliance
16. Encourage client and relatives to ask questions; gives appropriate answers
17. Record and report to the examiner
18. Thank patient and relatives
