It is a purposeful visit by the RCPN to the home of client and family to organize family therapy sessions. The nurse facilitates the sessions with respect to health needs and crisis identified in the family.
Aims
· Help mitigate crisis in the family and promote conducive home environment
· Promote effective communication in the family
· Understand and handle special family situations
· Ensure continuity of care and effective rehabilitation
Requirements
· A Well-Equipped home visit bag (refer to content)
· Community Map and or GhanaPost Global Positions System (GPS)
· Manual/electronic area register/ records of the client
· Means of Transportation
Steps
1. Read map of the community or use GhanaPost Global Positions System (GPS) address to identify the location of the centre
2. Announce your presence, enter on response, greet and introduce yourself to client, family and significant other
3. Establish rapport (refer to procedure) and enquire about the general health of family
4. Explain purpose of the therapy
5. Ensure privacy and confidentiality by asking the family to identify a suitable place for the session
6. Ensure the environment is clean, safe and conducive for the assessment
7. Observe the sitting position of the family members
8. Commence the session in a calm and respectful manner to gain family confidence and cooperation
9. Introduce the subject for the session
10. Encourage open communication amongst family members
11. Gather data regarding family history
12. Identify problems that concerns the family
13. Probe further to identify additional problems
14. Assess the various roles each member plays
15. Determine if the family roles are rigid or flexible
16. Suggest ways to resolve conflict if any
17. Formulate interventions to bring about positive change in the family
18. Refer problems beyond scope to the nearest mental health unit or health facility
19. Discuss with family and agree on a date for the next session
20. Leave your contact with family and encourage them to call when necessary
21. Document the care and findings in the report book
22. Express appreciation and depart
