This is a special visit made by the Public/Community health nurse to a client living with HIV/AIDS at home.
Aims
· Counsel client on adherence to prescribed medication to prevent drug resistance
· Counsel and educate client and family on personal hygiene and adequate nutrition to improve immune system
· Provide support for clients and their relatives or caregivers to prevent lapses in treatment
· Prevent opportunistic infections, disability and risk of death
· Reduce stigmatization
· Ensure continuity of care
· Counsel client and family on sexual habit
· Ensure proper infection prevention practices in the care of the client
· Liaise for support from available social systems
Requirements
· Refer to items in the well-equipped home visiting bag
· Map of the area/community
· Records of the client manually/electronically
· Register of the area/community
· Arrange for transportation
Steps
1. Read map of the community
2. Knock, enter, greet and introduce self
3. Establish rapport, provide privacy and ensure confidentiality
4. Observe environment for cleanliness, safety and comment
5. Ask about general health of client
6. Assist client to identify health needs
7. Check and use items in the home visit bag
8. Interview and counsel client on:
· Disease condition
· Schooling/occupation
· Diet
· personal health
· Sleep pattern and recreational activities
· Bowel action/hydration
· Medications and regular continuation of treatment
· Prevention of home accidents
· Relationship with family and community members
· Home help (caregiver)
· Prevention of re-infection (Safer sex practices)
· Other concerns and date for review
9. Congratulate client and family and schedule for next visit
10. Record and report findings manually/electronically
