Component Task: Admission of an Ambulant Child

This is a process of receiving a conscious/alert child into the hospital/ward. The child walks into the hospital aided/unaided.

Steps

1.        Welcome child and caregiver into the ward

2.      Establish rapport and make them comfortable

3.      Introduce staff present and reassure child and caregiver

4.      Confirm patient name and diagnosis from admission records (Manual or electronic)

5.      Send child to the prepared cot/bed with side rails

6.      Introduce other children and caregivers if necessary

7.       Take history and perform physical assessment

8.      Check vital signs, weight, height and record

9.      Administer urgent medication if prescribed

10.    Obtain necessary equipment for the child’s care

11.      Collect specimen if requested

12.    Assist child to change into pajamas or hospital clothing

13.    Orientate child and caregiver to the ward e.g. playroom

14.    Prepare the child/caregiver for discharge

15.    Explain the hospital/ward policies e.g. visiting time, National Health Insurance policies etc. to caregiver

16.    Arrange for a translator for the child’s care when needed

17.    Ensure caregiver/family sign consent form for treatment etc. if necessary

18.    Encourage caregiver to bring child’s play and learning materials

19.    Hand over child’s valuables to caregiver

20.  Update the ward record with patient’s information (manual or electronic)

21.    Document all procedures and observations (manual or electronic)
