Component Task: Administration of Tablet(s) to a Hypoactive Patient

This is the process by which the nurse administers/serves medication(s) in a tablet form through the mouth to a less active, withdrawn or retarded patient. The administration of a medicine is a common but important clinical procedure to treat and prevent illness.

Aims

·         Promote early recovery

·         Prevent medication errors and minimize complications

Requirements

a.      A tray containing the following:

·         Patient’s treatment chart

·         Disposable spoon

·         A jug of water

·         Plastic cup of water on a plastic saucer

·         Tissue paper

·         Patient’s medication(s)

·         Pill crusher

b.      Patient’s folder (Electronic/Manual)

c.      Assisting nurse(s)

Steps

1.        Establish rapport (refer procedure) and explain procedure to the patient

2.      Identify and check for the rights of medication administration (e.g. right patient, right tablet(s), right to refuse, right documentation, etc.)

3.      Perform hand hygiene

4.      Prepare tray and send to patient’s bedside or appropriate place

5.      Check the prescription again with the medication

6.      Demonstrate an attitude of interest and warmth towards the patient

7.       Speak gently and calmly to the patient throughout the procedure using words of encouragement

8.      Open the container/blister and pick tablet(s) with a spoon ensuring the right medication and dosage

9.      Suggest to and assist patient to tilt head backward and support the head with the help of another nurse

10.    Coax patient to open the mouth using minimum force if necessary

11.      Pour some water into the mouth followed by the tablet(s) and encourage patient to swallow OR crush the tablet (s) per the manufacturer’s instruction and mix with water and administer to patient

12.    Rub the front of the neck gently to stimulate the swallowing reflex and ensure that the patient swallows the tablet(s)

13.    Put patient in a desired position

14.    Express appreciation to patient and direct him/her to the next activity

15.    Clear tray and perform hand hygiene

16.    Sign the treatment chart and document procedure in nurses’ notes/ patient’s folder  

17.    Observe patient for the effects of the medication and report
