This is the process of giving medication to a child with the aid of age appropriate size intravenous cannula through a vein for therapeutic purposes.
Aims
· Achieve faster absorption of medication
· Treat disease condition
· Administer relatively larger volume of medication/boluses and intravenous fluid
· Investigative purposes
Requirements
a. Set up a tray with the following items
· Correct medication
· Syringes and needles (appropriate size)
· Intravenous cannula (appropriate size)
· Alcohol swab/cotton swabs and methylated spirit
· Gallipot
· Receiver
· Plaster/adhesive skin tape
b. Pen
c. Paper/electronic health chart
d. Medication charts
e. Observation chart/adverse drug reaction chart
f. Normal saline
g. Safety sharp box
h. Sterile gloves
i. IV tubbing stopper/cap
Steps
1. Receive child/caregiver into a comfortable seat
2. Perform five (5) moments of hand hygiene as and when necessary
3. Introduce self to child/caregiver
4. Establish rapport (refer steps)
5. Explain procedure to child/caregiver (refer steps)
6. Incorporate developmentally appropriate strategies in dealing with the child
7. Verify the medication order with the child’s medical records
8. Ask child/caregiver about medication/allergy history and intervene as necessary
9. Provide privacy
10. Observe the Rights of Medication administration
· Right medication
· Right dose
· Right patient/client
· Right time
· Right route
· Right reason
· Right of child/caregiver to refuse medication
· Right documentation
11. Provide age-appropriate diversional therapy
12. Check the accuracy of the dose to be given to child
13. Assess for patency of IV access
14. Reset IV line if extravasation (tissue infiltration) has occurred
15. Reconstitute medication if required
16. Do not mix different medications in one syringe
17. Use the right size of syringe and needle to draw up the medication into the syringe; keep charged
18. Calculate the medication infusion rate/medication dose as prescribed
19. Assist child to assume the right position with caregiver support
20. Support the child to stay calm and expose the body part with the IV access
21. Don gloves and disengage the IV stopper from the IV cannula and keep secured
22. Administer IV medication/medication infusion slowly at the required rate
23. Observe for adverse drug reaction during the medication administration and intervene accordingly
24. Flush the IV cannula slowly with small amount of normal saline in syringe
25. Disengage the syringe from the IV cannula
26. Recap the IV cannula/reconnect to the IV tubing after medication
27. Re-tape the skin at the IV site with adhesive if necessary
28. Dispose off needle and syringe into sharp box/container
29. Discard tray and decontaminate as appropriate
