This is an activity carried out by the Public/Community health nurse to record the biodata of the child during the first visit.
Aims
· Serves as a baseline to monitor the growth and development of a child
· Keep records of clients for future reference
· Trace defaulters
· Promote continuity of care
· Maintain data for health planning

· 0-11 months
· 12-23 months
· 24-59 months
Requirements
· Furniture which includes chairs, tables, bulletin board
· Table cloth
· Two (2) registers, one for registration of children under one (1) year and the other for children above one year
· Master chart for grading nutritional status
· Attendance book
· Tally sheets
· Hand washing facilities- veronica bucket, plastic bowls, soap in a dish, hand towels
· Blue and red pens, pencils, eraser and ruler
Steps
1. Welcome mother/caregiver and offer a seat
2. Introduce self
3. Enquire about the health of the child and family
4. Take child’s history and fills the child health booklet
5. Register the child in the registration book and give registration and serial numbers
6. Enter the needed information in the registration book
7. Determine whether the child is due for immunization
8. Thank mother/caregiver for her cooperation
9. Refer client to the next stage
