This is a visit made by the Public/Community health nurse to a client with Yaws in his/her home.
Aims
· Educate client and family on how to manage the condition at home
· Encourage client and family to adhere to treatment i.e. dressing of ulcers
· Educate client and family on adequate nutrition
· Encourage client and family on follow up visit
· Educate client on personal and environmental hygiene

Steps
1. Read map of the community
2. Knock, enter, greet and introduce self
3. Establish rapport and ask about general health
4. Observe environment for cleanliness, safety and comment
5. Ensure privacy and confidentiality
6. Assist client to identify health needs
7. Assess client wound and plan care
8. Check and use items in home visit bag
9. Wash hands/alcohol hand rub with appropriate technique
10. Dress wound

12. Interview and counsel on:
· Health education on disease condition
· Diet
· Exercise/rehabilitation
· Sleep pattern and recreation
· Medication and regular check-ups
