Component Task: Home Visit to A Client With Cerebrovascular Accident (CVA)

This is a visit made by the Public/Community health nurse to a CVA client at his/her home.

·         Provide continuous care to client and family

·         Counsel/educate client and family on management of the condition

·         Counsel client on diet modification and adherence to treatment

·         Sensitize client and family on rehabilitation (speech and physiotherapy)

·         Map of the area/community

·         Records of the client manually/electronically

·         Arrange for transportation

Steps

1.      Read map of the community

2.      Knock, enter, greet and introduce self

3.      Explain the purpose of the visit and ensure confidentiality

4.      Observe environment for cleanliness, safety and comment

5.      Enquire about the general condition from client/caretaker/relatives

6.      Assist client to identify health needs

7.       Check and use items in the home visit bag

8.      Wash hands/alcohol hand rub with appropriate technique

9.      Check Blood Pressure (BP) and compare with the previous one (if available)

10.    Undress the client with assistance from the caretaker

11.      Examine client from head to toe

12.    Wash hands/alcohol hand rub with appropriate technique

13.    Interview and counsel on:

·         Findings to caretaker

·         Personal health with emphasis on pressure areas

·         Frequent change of position and exercise d. Healthy diet

·         Medication and regular check-ups

14.    Ask of other concerns

15.    Schedule for next visit and thank them

16.    Record and report findings manually/electronically
